Post-Surgery Recovery Companion App with Smartwatch Integration

Let's Connect

Overview

What we built

An orthopaedic implant maker had no idea what happened to joint-replacement patients between surgery and the six-week review. We built the smartwatch-paired app that gave care teams a daily view of recovery instead of a six-week guess.

In plain terms: patients recovering from joint replacement went home with a printed sheet of exercises and were not seen again until the six-week review. In between, nobody at the 25 partner surgical centres piloting this programme knew whether a patient was walking, doing the prescribed exercises, or struggling in silence. Clinic staff tried to fill the gap with reassurance calls, but those calls went out to everyone rather than to the patients who actually needed one.

We built iOS and Android companion apps paired with each patient's smartwatch, turning the printed sheet into a day-by-day plan of step goals, guided exercise videos and pain check-ins, with step counts syncing automatically from the watch. A clinician dashboard flags patients falling behind their expected recovery curve, so care teams call the right people first. Daily adherence averaged 76% across the 25-centre pilot, and routine phone check-ins fell by 38% as real check-in data replaced blanket calling.

The Problem

Recovery invisible between visits

Joint-replacement recovery used to go dark the moment a patient left the surgical centre. A printed sheet of exercises was the only guidance sent home with them, and the next scheduled contact was the six-week review, an enormous window in which a patient could be recovering well, quietly falling behind, or moving the wrong way, and nobody at the centre would know which.

Care teams tried to close the gap the only way they had: routine reassurance calls. But those calls were not targeted, so staff time went into checking on patients who were recovering fine just as often as it reached the ones who were struggling. The patients most at risk of falling behind were exactly the ones a blanket calling schedule was least likely to catch in time.

Across the 25 partner surgical centres piloting the programme, this pattern repeated at scale: printed sheets, a six-week blind spot, and clinic staff spending hours on calls that could not tell them who actually needed to hear from them.

Six-week blind spot

Patients went home with a printed exercise sheet and were not seen again until the six-week review, leaving recovery entirely unmonitored in between.

No movement visibility

Care teams had no way to see whether a patient was walking, exercising or struggling at home, only what the patient reported at the next visit.

Untargeted reassurance calls

Routine phone check-ins went out to every patient regardless of how they were actually recovering, consuming staff time without reliably reaching those in trouble.

Scaling across 25 centres

The same blind spot repeated at every one of the 25 partner surgical centres piloting the programme, multiplying the risk across the whole cohort.

What it was costing them

Every patient sent home with only a printed sheet was a recovery care teams could not actually see, across all 25 partner centres in the pilot. Reassurance calls consumed clinic staff time without reliably reaching the patients who needed help most, and a struggling patient could go unnoticed for weeks until the six-week review finally revealed a problem that earlier attention might have prevented.

The Solution

Guided recovery companion app

We built iOS and Android companion apps paired directly with the patient's own smartwatch, replacing the printed exercise sheet with a day-by-day recovery plan. Step goals, guided exercise videos and pain and symptom check-ins gave patients a clear, structured routine to follow from the day they left the surgical centre, rather than a single sheet of instructions to interpret alone.

Step counts sync automatically from the watch, so the app captures real movement data without asking the patient to log anything by hand. That data feeds a clinician dashboard that flags patients falling behind their expected recovery curve, turning the six-week blind spot into a picture care teams can check daily across all 25 partner centres.

The dashboard changed how reassurance calls worked in practice. Instead of calling every patient on a fixed schedule, care teams could call the ones the data actually flagged as falling behind, so staff time went where it was needed rather than being spread evenly across everyone.

Key decisions

01

Pair with the patient's smartwatch

The app paired directly with each patient's own smartwatch rather than requiring a new device, so step data started flowing from day one after surgery.

02

Day-by-day recovery plan

A day-by-day plan of step goals, exercise videos and check-ins replaced the single printed sheet, giving patients structure rather than one static instruction set.

03

Automatic step syncing

Step counts sync automatically from the watch, capturing real movement data without relying on the patient to log anything themselves.

04

Clinician dashboard with flags

A clinician dashboard flags patients falling behind their expected recovery curve, replacing the six-week review as the first point care teams learn of a problem.

05

Targeted, not blanket, calling

Care teams could call the patients the dashboard actually flagged, replacing routine reassurance calls that reached everyone regardless of need.

Measurable Impact

What changed after launch

Daily exercise plan adherence averaged 76% across the 25-centre pilot cohort, and 87% of enrolled patients logged activity at least 5 days a week in their first month, evidence that a day-by-day plan paired with a smartwatch gets followed far more consistently than a printed sheet ever did.

Routine care-team phone check-ins fell by 38% as real check-in data replaced blanket calling, freeing staff time for the patients the dashboard actually flagged. The app is rated 4.6 stars by patients, with 92% still active at week 6, right at the point the six-week review used to be the first real check on recovery.

Recovery visibility

No contact between surgery and the six-week review

Daily adherence averaged 76% across 25 centres

Patient engagement

A single printed exercise sheet sent home

87% logging activity 5 days a week in month one

Care-team calling

Routine reassurance calls sent to every patient

Phone check-ins down 38% with flagged calling only

App experience

No digital recovery support at all

Rated 4.6 stars, 92% still active at week 6

Tech & Tools Used

What powered the build

Every tool below earned its place in this engagement. Here is the part each one played.

React Native logo

React Native

Powered the iOS and Android companion app from one codebase, delivering the day-by-day recovery plan and check-ins to patients at all 25 partner centres.

TypeScript logo

TypeScript

Typed the app and backend code shared across the recovery plan, exercise video and check-in features, keeping both platforms consistent as the pilot scaled.

Node.js (NestJS) logo

Node.js (NestJS)

Served the API behind the clinician dashboard and the recovery plan logic, including the flags that surface patients falling behind their expected curve.

PostgreSQL logo

PostgreSQL

Stored patient recovery plans, step history and check-in responses, giving the clinician dashboard one consistent source for every flag it raises.

Apple HealthKit logo

Apple HealthKit

Pulled automatic step counts from iPhone-paired smartwatches, feeding the recovery plan without requiring patients to log activity by hand.

Google Fit API logo

Google Fit API

Synced step data from Android-paired smartwatches, keeping automatic activity syncing consistent for patients on either platform.

Firebase Cloud Messaging logo

Firebase Cloud Messaging

Delivered daily exercise reminders and check-in prompts, keeping the recovery plan active on a patient's phone between sessions.

AWS (ECS + RDS)

Hosted the backend services and database behind the clinician dashboard, running the pilot reliably across all 25 partner surgical centres.

Metabase logo

Metabase

Powered the reporting behind the adherence and engagement figures the pilot used to track patients falling behind their recovery curve.
