Automated Recall and Recovery Engine for a Family Dental Group

Let's Connect

Overview

What we built

A 32-office family dental group had let its recall list swell to roughly 40,000 patients overdue for hygiene visits, with front desks too busy to work it. We built an automated recall and recovery engine, and the hygiene reappointment rate rose from 59% to 76% within 8 months.

In plain terms: across the group's 32 offices, roughly 40,000 patients were overdue for a hygiene visit and sitting untouched on a recall list. Front desks were fully occupied checking in the patients already walking through the door, so nobody had time to work the overdue list. Postcard reminders went out and mostly went unanswered, a cancelled appointment simply vanished from the schedule rather than being offered to anyone else, and hygiene chairs sat empty while the group spent money acquiring brand-new patients instead of recovering the ones it already had.

We built an automated recall and recovery engine on top of the group's practice-management system. Overdue patients now enter segmented SMS and email cadences with one-tap online rebooking, last-minute cancellations trigger instant offers to a standby list instead of vanishing, and unscheduled treatment plans get their own follow-up sequences, all visible through a centralised dashboard covering every office. The hygiene reappointment rate rose from 59% to 76% across all 32 offices within 8 months, 6,400 of the 40,000-patient backlog rebooked in the first 6 months, and outbound recall calls by front-desk staff fell by roughly 70%.

The Problem

Overdue patients nobody could call

The group's recall list, the patients overdue for a routine hygiene visit, had swollen to roughly 40,000 names across its 32 offices before anyone treated it as a priority. Front desks were fully occupied checking in the patients already in the building, so working through a list that size by hand was never realistic.

The reminders that did go out were postcards, and most went unanswered, sitting on a fridge rather than prompting a booking. When a patient cancelled at short notice, that slot simply vanished from the schedule instead of being offered to someone else waiting for an earlier appointment.

The result was hygiene chairs sitting empty across the network while the group spent money acquiring brand-new patients rather than recovering the roughly 40,000 already on its books. Front-desk staff, meanwhile, spent real time on outbound recall calls that rarely converted.

40,000-patient recall backlog

The recall list of patients overdue for a hygiene visit had swollen to roughly 40,000 names across the group's 32 offices before anyone could work through it.

Front desks too busy to call

Staff were fully occupied checking in patients already in the building, leaving no time to work the growing overdue list at any of the 32 offices.

Cancellations simply vanished

A last-minute cancellation freed a slot that nobody else was offered, so the appointment vanished from the schedule instead of being filled.

Chairs empty, new patients bought

Hygiene chairs sat empty across the network while the group spent money acquiring brand-new patients instead of recovering the ones already overdue.

What it was costing them

Roughly 40,000 overdue patients represented hygiene visits, and revenue, the group was not collecting, while front desks stayed too busy checking patients in to work the list. Postcard reminders rarely converted, cancelled slots vanished instead of being refilled, and the group kept paying to acquire brand-new patients even as chairs sat empty across all 32 offices.

The Solution

Automated recall and recovery engine

We built an automated recall and recovery engine on top of the group's existing practice-management system, so the roughly 40,000-name backlog finally had somewhere automated to go. Overdue patients now enter segmented SMS and email cadences instead of waiting on a postcard, with one-tap online rebooking so a reply turns straight into an appointment.

Last-minute cancellations no longer simply vanish: they trigger instant offers to a standby list of patients waiting for an earlier slot, so a gap in the schedule gets filled automatically. Unscheduled treatment plans get their own follow-up sequences, chasing the work that had been recommended but never booked.

A centralised dashboard shows recovery performance for every one of the 32 offices, so the group can see which locations are working through their backlog and which still need attention.

Key decisions

01

Segment the recall list

Overdue patients enter segmented SMS and email cadences rather than one generic postcard, so the message matches how overdue and how urgent each patient's visit is.

02

One-tap online rebooking

Every reminder links straight to one-tap online rebooking, so a patient replying to a message can book an appointment without a phone call.

03

Fill cancellations with a standby list

Last-minute cancellations now trigger instant offers to a standby list, so a slot that would have vanished gets filled automatically instead.

04

Chase unscheduled treatment too

Unscheduled treatment plans get their own follow-up sequence, recovering recommended work that had never been booked in the first place.

05

Centralise performance across offices

A single dashboard shows recall and recovery performance for all 32 offices, giving the group one place to see where the backlog is shrinking.

Measurable Impact

What changed after launch

The hygiene reappointment rate rose from 59% to 76% across all 32 offices within 8 months, and 6,400 patients from the roughly 40,000-name backlog were rebooked in the first 6 months alone. Cancelled hygiene slots stopped simply vanishing: 43% now refill the same week through automated standby-list offers.

Front-desk staff felt the change directly, with outbound recall calls reduced by roughly 70%, freeing them to focus on the patients already in the office rather than working through a list by phone.

Tech & Tools Used

What powered the build

Every tool below earned its place in this engagement. Here is the part each one played.

Next.js logo

Next.js

Powers the centralised dashboard showing recall and recovery performance across all 32 offices, and the one-tap rebooking pages patients land on.

Node.js logo

Node.js

Runs the recall engine's core service, segmenting overdue patients and triggering the right SMS, email or standby-list cadence for each one.

PostgreSQL logo

PostgreSQL

Stores the recall list, appointment history and standby-list records that now cover roughly 40,000 patients across the group.

Redis logo

Redis

Holds fast-moving state such as standby-list positions, so a cancelled slot can be offered out within moments of opening up.

BullMQ

Schedules and retries the segmented SMS and email cadences and follow-up sequences behind the recall and recovery engine.

Twilio SMS logo

Twilio SMS

Delivers the SMS side of the recall cadences and the instant standby-list offers triggered by last-minute cancellations.

SendGrid logo

SendGrid

Delivers the email side of the recall cadences, reaching patients who prefer email over a text message.

Docker logo

Docker

Packages the recall engine's services so they run consistently across the group's environments as usage scaled with the backlog.

AWS ECS

Hosts the recall and recovery engine in production, running the services behind all 32 offices' recall cadences.
